Case Management


Dual Diagnosis Case Management:

Provides intensive case management support geared to meet the unique needs of the Dual Diagnosis clients. The emphasis is building a strong partnership within the community to assist clients to discover their strengths, determine area of need and develop support plans. This may include skills development, planning daily/weekly activities, accessing medical services, grocery shopping, grooming and hygiene, and dealing with income and housing issues. For people with limited support in community

Eligibility: Individuals who are residents of Barrie and 16 + who are experiencing serious mental illness and have been diagnosed with developmental disability and is receiving limited support in the community. Processing will occur through central intake.

Family Support Case Management:

Work collaboratively with family members, care-givers, clients, peers and the community to enhance the physical and mental health of persons with serious mental illness. This service will provide individual supportive counseling, group educational opportunities and referrals to appropriate community supports and resources.

Eligibility: family members, caregivers, friends and significant others of individuals who live with a mental illness that may or may not be receiving services from this agency.
