In Ontario Health Central Region, referrals for the OSP program are being coordinated by Waypoint Centre for Mental Health Care, and services are being delivered in a wide range of community settings. This includes primary care, community mental health agencies, post-secondary institutions, Indigenous organizations, and family services agencies.
If you are a psychiatrist, a primary care provider (family doctor, nurse practitioner), or other health care professional, you can refer clients into the OSP program. Alternatively, clients may sign up for the program themselves.
A referral or self-referral can be made by completing and submitting the following online referral form. If the person being referred does not have access to email, a completed faxed form can also be sent to 705-527-0390. Click here to find printable versions of the questionnaires.
