North Simcoe Muskoka Transgender Health services provides culturally and gender-affirming care to trans and gender diverse people and their families within the North Simcoe Muskoka region. You may self-refer. No physician referral is required.

Indigenous Case Management
The Case Management Team includes an Indigenous Case Manager who provides support, service navigation, and case management for individuals who self-identify as Indigenous. This role also includes outreach and collaboration with Indigenous agencies and community resources. Anyone can make a referral including self-referral.
Indigenous Navigator – Crisis Intervention
The Crisis Intervention and Community Response Program goes beyond front-line crisis intervention by integrating a multi-disciplinary team that delivers a range of specialized services, including: Indigenous Navigator.
Family Support Program
Our family support staff provide skills teaching, system navigation, and support to family members and caregivers whose loved one experiences mental health and/or addiction concerns across Simcoe County.
